Why this shape

Failures are decision failures.

When systems fail in significant ways, the cause is almost never that someone could not write the necessary code. It is that a decision was made under unclear conditions, or made by no one in particular.

The structure of the firm — small, direct, slow to delegate — exists to keep the quality of decisions high enough to trust. The same standard is applied to a one-day correction and to an engagement that runs for years. The size of the work changes how long it takes; it does not change how it is reasoned about, or who is accountable for the result.
